GLP-1 receptor agonists such as semaglutide (Ozempic, Wegovy) and liraglutide (Saxenda) are administered via a pre-filled, disposable KwikPen. The pen is simple to use, requires no mixing, and delivers a precise subcutaneous dose — typically once weekly or once daily depending on the agent.

Am I eligible for GLP-1 treatment?

Eligibility is determined during your initial in-person consultation with Dr Mothapo. Generally, GLP-1 therapy is considered for adults with a BMI of 30 or above, or a BMI of 27 or above in the presence of a weight-related condition such as type 2 diabetes, hypertension, or sleep apnoea. A full medical history, examination, and blood panel are completed before any prescription is issued — there are no shortcuts.

Are GLP-1 medications safe?

The GLP-1 receptor agonists we prescribe — semaglutide and tirzepatide — are SAHPRA-approved, extensively studied, and non-habit forming. They have been used in clinical practice for over a decade. As with any medication, there are contraindications and potential side effects, which is why our prescriptions are preceded by thorough assessment and followed by regular medical reviews.

What side effects can I expect?

The most common side effects are gastrointestinal — nausea, mild constipation, or reduced appetite — and are most pronounced in the first few weeks as your body adjusts. These are managed through careful dose titration: we start low and increase slowly. Most patients find that symptoms resolve within two to four weeks. If they don't, we adjust your dose or management plan. You are never left to manage side effects alone.

Do I need medication to join the programme?

No. Medication is one tool — not a requirement. Patients who prefer, or who are not candidates for, GLP-1 therapy can engage fully with our dietetic and psychological support pathways. A significant number of our patients achieve meaningful, sustained results through nutrition, behavioural coaching, and lifestyle restructuring alone. Your programme is built around your needs, not a fixed formula.

What does the initial consultation involve?

Your first appointment is a comprehensive 60-minute assessment with Dr Mothapo. This includes a full medical history, BMI and body composition review, cardiovascular and metabolic screening, and discussion of your weight history, goals, and previous treatment attempts. Where indicated, a blood panel is ordered. From this, a personalised treatment plan — spanning medical, dietetic, and psychological support — is developed and discussed with you in full.

How is Beyond the Scale different from a GP or dietician alone?

Obesity is a complex, multisystem disease. A doctor alone manages the medical dimension; a dietician alone addresses nutrition; a psychologist alone works on behaviour. At Beyond the Scale, all three disciplines collaborate on a single shared plan — and they communicate with each other, not just with you. This integration is what makes outcomes different. It is the same standard of care recommended in the 2025 international obesity treatment guidelines.

What happens when I stop GLP-1 medication?

Stopping abruptly is not recommended. Dr Mothapo develops a gradual tapering plan that gives your metabolic systems time to adapt, reducing the risk of rebound weight gain. This is supported simultaneously by your dietetic and psychological teams, who strengthen the behavioural and nutritional foundations that will sustain your results long after medication is discontinued.

Can I book a telehealth appointment?

Yes. Follow-up medical reviews, GLP-1 dose reviews, dietetic consultations, and psychology sessions can all be conducted via secure video call through the patient portal. Your initial assessment with Dr Mothapo is in person at our Mbombela rooms — this allows us to complete the physical examination and baseline investigations that a screen cannot replace. All subsequent appointments can be in-person or telehealth, depending on your preference and clinical need.
